Only available to T-Mobile Magenta, T-Mobile Magenta Plus, T-Mobile Military, and T-Mobile Magenta 55+ voice lines activated through a BMP promotion and incur a customer-facing monthly recurring charge (each, an “Activated User Line”). See T-Mobile plan details at www.t-mobile.com. New paid Activated User Line required, existing lines moving to plan do not qualify. Only available when purchasing via the T-Mobile Advantage Program telesales team, the promotion is not available in T-Mobile retail locations, online or through any other channel.

Users shall (i) be subject to and must satisfy T-Mobile’s credit requirements; (ii) execute a form of T-Mobile’s Service Agreement and agree to be bound by the T-Mobile Terms and Conditions of Service; and (iii) comply with T-Mobile’s activation procedures. For each Activated User Line, to participate in the promotion Users shall be required to complete, within 120 days of activation, a promotion rebate form at www.bmprebates.com, and provide their e-mail address, T-Mobile order number, promo code and/or other reasonable identification as confirmation of eligibility.

Users may be required to activate Service for a certain rate plan or feature or to sign an agreement to finance equipment that begins on the date such Users sign a Service Agreement and such term(s) may continue past the termination date of the promotion.

The Activated User Line must be in active and good status, and eligibility requires that no other preexisting lines on a T-Mobile account (i) are suspended or disconnected, or (ii) have been cancelled and replaced with an Activated User Line within 6 months of the cancellation. Maximum of 12 Activated User Lines per User.
